Sudden Infant Death Syndrome [SIDS]
Public Health Nurse Case Management Services
Goal:
- To increase community understanding of SIDS.
- To provide support and information to affected families and care givers.
- Gather data for the State Department of Health Services.
Services Provided:
- Families or care givers who have experienced a SIDS loss receive a home visit from a Public Health Nurse (PRN) within three working days of agency notification of the loss
Visit includes:
- Assessment of counseling, support and education needs of family
- PHN provides information about SIDS, support and initial grief counseling
- Pertinent SIDS related data is gathered
- Plan for further services is developed
Families receive continued case management as their needs require. Information and education is provided to the community at large and parents of infants in particular about SIDS, associated risk factors and the "Back to Sleep" campaign.

Target Population: Parents, families and other care givers who have experienced the death of an infant due to SIDS. Community members requiring information about SIDS.

Place your baby on his/her back to sleep, breast feed, avoid exposure to secondhand smoke.
